Initial Visit Forms

Dear New Patient,

Please click on both of the links below to print your initial evaluation forms.  Bring the completed forms with you to your first visit. 

We anticipate your initial evaluation will take about an hour. Please do not eat or drink anything except water for 2 hours prior to this appointment. You will need to be well hydrated for testing so please do drink water during these 2 hours. Also, for testing accuracy and as a courtesy to our more sensitive patients, we are a fragrance free office so thank you for avoiding wearing colognes, perfumes, or other scented body products to our office.
